MAJOR FUNCTION: Physician Leader responsible for the delivery of participant care and clinical outcomes along with the Interdisciplinary Team (IDT).  Instrumental in the implementation and oversight of the Quality Assessment and Performance Improvement (QAPI) program.  Responsible for the development and maintenance of medical policies and procedures/protocols. Provides direction, collaboration and supervision to related primary care providers, nursing and allied health services. Performs some direct patient care as a member of the primary care provider team.  Responsible for Sentinel reporting process for LIFE.





A.  Education:                         Medical Doctorate Degree: MD or DO                       




B.  Experience: Minimum of 2 years working with the frail or elderly                    



C.  Certification/Licensure: Valid Driver’s license, proof of insurance and have a reliable means of

transportation as described in Fleet Policy # TR 118. 

Maintains valid NJ Medical License, CDS/DEA License and Active Board Certification in area of specialty.
