Coordinates the overall path of care for patients with a diagnosis of heart failure. The clinical care coordinator will serve as a navigator and will provide comprehensive coordination of care for patients and their families/caregivers who are undergoing diagnosis, treatment, and follow-up for heart failure. The duties of the clinician/navigator will include but not be limited to working with clinicians to ensure standard of care treatment/therapy is initiated and/or continued, educating, supporting, and assisting patients and their families, serving as a link to their providers and support services. The coordinator/navigator will help to assist with interdisciplinary care coordination; and collaborate with colleagues to develop performance indicators in accordance with organizational goals.
QUALIFICATIONS:
Education & Experience:
- Graduate of an approved RN nursing program, BSN required.
- MSN preferred.
- 5 years of experience as a registered nurse, with recent cardiology experience preferred in an acute care, outpatient, or cardiology practice setting.
Certification/Licensure:
- Valid New Jersey RN License.
- BLS required.
