RN TRANSITIONAL CARE - INSPIRA LIFE CENTERS

MAJOR FUNCTION: Under the supervision of the Director of Nursing, manages the care of participants to provide high quality, cost-effective care. Coordinates services with primary care team. Triages and assesses participants as needed. Acts as a gatekeeper for participants in all settings and levels of care. Provides participant, caregiver, family and staff teaching and communicates participant's condition to caregivers, primary care, and the interdisciplinary team. Acts as a liaison for effective communication between the LIFE center and contract facilities. Coordinates care with the Interdisciplinary Team and contracted facilities to provide services, as outlined in the Plan of Care. Coordinates end of life care so participants receive the appropriate medical, pharmaceutical, and psychosocial services.

As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.

QUALIFICATIONS:

A. Education:

Graduate of a school of professional nursing. BSN preferred. Palliative care experience preferred.

B. Experience:
  • Minimum of 1 year working with the frail or elderly
  • 2 years of Nursing Experience

C. Certification/Licensure: Current BLS (American Heart Association). Certification in appropriate specialty preferred.
