Social Worker - Population Health BC

MAJOR FUNCTION: The Population Health Social Worker provides medical, psychological and/or social service assessments on patients falling under the purview of the Population Health Department. The social worker serves as a patient advocate on both a strategic and tactical basis. The social worker authors psycho-social assessments, develops individualized care plans and engages the patient in actualizing the plan. In addition, the social worker identifies community and government resources, including grant opportunities, that can used to support the healthcare, social and economic needs of patients. The social worker actively collaborates with the department's nurse care managers, Inspira Health Network providers and administrative staff to ensure a coordinated, cost effective and wholistic approach to the delivery of healthcare services. The social worker also provides support and guidance to the department's patient engagement specialists.

QUALIFICATIONS:

A. Education: Masters in Social Work/Licensed Clinical Social Worker.

B. Experience:
3-5 years in a health care setting or community service organization.
